Technical Service Request (TSR) Form
Please
complete the TSR form to request computer repairs or components.
Place the TSR form in my mailbox in the main office. Your request
will be processed, as soon as possible.
VERIFY
Student Management System
If
you have a working computer in your classroom, please complete the
TSR (Technical Service Request) form to get VERIFY installed on the
computer. Click the TSR button, on the left of this page, to get
a form.
The
installation of VERIFY on classroom computers will begin by April
1.
Quick
Tips: If the VERIFY icon is missing from the computer desktop,
look in Programs for VERIFY.
To
enter absences for a previous date, click in the drop down box near
date and select the desired date from the calendar. Then click UPDATE
FILES to see the list of your classes.
Non-Functioning
Computers
Please
complete the TSR form to request the removal of non-functioning computers
in your classroom. If you need a part for the computer (such as,
keyboard, mouse, Internet cable, etc.), please indicate so.
Software
Installation
If
you have software to be installed, please complete the TSR form and
provide the software to me in Room 218A.
